Preparing for your first appointment at Momentum Health

We will have emailed you a link to a fillable intake form. Save time on your first visit by completing your intake paperwork online, prior to your appointment. If you do not have time to complete them ahead of time, you should arrive at least 30 minutes early to give yourself time to finish them before your appointment.

The Momentum Health team is committed to finding the root cause of your condition and developing a treatment plan that addresses your needs and helps you achieve your goals. Completing your intake paperwork, which includes a detailed past and current medical history, assists our clinicians in accomplishing this.

Payment Options

Before your visit, we recommend you call your insurance company to verify your insurance benefits. If you have been injured at work you may qualify for [WCB] coverage. If you’ve been involved in a vehicle collision, you may have coverage under [motor vehicle accident] Section B insurance.

Direct Billing

We bill directly to your insurance for your convenience

Direct billing simplifies the process of paying for your services. Instead of paying out of pocket and waiting for reimbursement, we can bill your extended healthcare benefits company directly, saving you time and hassle.

How Direct Billing Works

When you visit our clinic, we will verify your insurance coverage and bill your insurance company directly for the cost of your treatment. You will only be responsible for any portion of the cost not covered by your insurance plan.

Insurance Benefits

We Direct Bill to over 20 insurance providers. You may have coverage under your employee benefits or other third party providers.

Types of Insurance Coverage:

  • Health Insurance: Many health insurance plans cover physiotherapy, chiropractic, massage therapy, registered dietician, psychology services, orthotics, and custom bracing. Health Spending Accounts often cover physician services not covered under Alberta Health Services. Check with your insurance provider to see if these services are included in your plan.
  • Motor Vehicle Accident Insurance: If you’ve been injured in a car accident, your Section B auto insurance may cover the cost of physiotherapy, chiropractic, massage therapy, and psychology services. Please follow this [link] for more information regarding motor vehicle accident injuries.
  • Workers’ Compensation (WCB): If you’ve been injured at work, Workers’ Compensation Board Insurance may cover the cost of physiotherapy or chiropractic treatments. Please follow this [link] for more information regarding work injuries.

All Albertans involved in a car accident/collision, regardless of who is at fault, are covered under Section B Medical Payments coverage of the Automobile Accident Insurance Benefits Regulation (AAIBR), and the Alberta Diagnostic and Treatment Protocols DTPR.

What should you do first after a vehicle collision in Calgary?

Car accidents are a leading cause of death and injury in Alberta and can cause serious injuries, including whiplash, spinal cord injuries, brain injuries, broken bones and internal bleeding. If you are experiencing any severe symptoms head to the nearest Emergency Department.

Injuries may also be minor, and you may notice some aches and pains after the initial shock of the collision wears off but symptoms can continue to worsen for several days after the accident. Just because these whiplash injuries are labeled as ‘minor’ does not mean you do not need to obtain treatment to resolve the injury in a timely fashion.

FAQs

Can all physiotherapists order x-rays and ultrasound Imaging?

Physiotherapists who have taken additional training are authorized to refer patients for x-rays and other imaging. These tests are used conservatively to assist in establishing a diagnosis, to assist in interventional procedures, or in clinical decision making. All Momentum Health & Evidence Sport and Spine clinics have clinicians who have been authorized by the College of Physiotherapists of Alberta to order diagnostic imaging.

Should I continue seeing a therapist even when I feel better?

The short answer is yes! However, at [site-name], we encourage you to continue seeing your physiotherapist after your symptoms appear to have resolved, if it is medically warranted.

Even though you may feel better, you may still not be fully rehabilitated. Furthermore, regular physiotherapy can prevent future injury or pain while helping you optimize your overall physical performance and function.

Physiotherapy is a comprehensive and ongoing process, not a one-and-done. Even after you’ve completed your initial round of treatment, our therapists can help you develop a personalized medical plan that ensures you’re continuing to maximize your potential.

Here’s an example:

  • Your therapist assigns you a series of specialized at-home exercises so you can continue to strengthen your muscles and improve your range of motion.
  • You come in for a re-evaluation after a period of time, to ensure your home exercise program is working as intended.
  • You schedule check-in appointments at regular intervals (such as every six months) to ensure you haven’t regressed and to address any new developments.

Our clinicians will also develop a return-to-activity schedule so you can safely return to your favourite sport or workout program while minimizing the risk of reinjury. A return-to-activity schedule involves gradually increasing the intensity of certain movements until you can operate as usual.

How can I pay for each session?

For any services we are not able to directly bill to your insurance, our policy is to collect payment at the time of service to ensure a seamless experience for all patients. We also provide payment options based on your unique circumstances and accept various methods of payment, including the following:

  • E-transfer
  • Credit card
  • Debit card

If you have insurance, please contact your provider to better understand your coverage, copayments/coinsurance, and overall plan requirements. Our staff will also assess your benefits and inform you of your payments at each visit.
