We bill directly to your insurance for your convenience
Direct billing simplifies the process of paying for your services. Instead of paying out of pocket and waiting for reimbursement, we can bill your extended healthcare benefits company directly, saving you time and hassle.
How Direct Billing Works
When you visit our clinic, we will bill your insurance company directly for the cost of your treatment. You will only be responsible for any portion of the cost not covered by your insurance plan.
Simply provide us with your insurance information when you book your appointment, and we’ll take care of the rest. Be sure to bring your insurance card and any relevant details about your coverage to your appointment. We are able to provide you with your receipts.
Benefits of Direct Billing:
- Convenience: You don’t have to pay out of pocket and wait for reimbursement.
- Time-saving: Direct billing saves you time and effort.
- Cost-effective: Direct billing ensures that you only pay your portion of the treatment cost, if any.

How much will my visits cost?
Our prices are posted on each locations’ unique online booking portal. Each discipline and service is listed separately. The total cost of care depends on your individual needs and goals. We will tailor our treatment plans to you, so the number of recommended visits varies. Your clinician will discuss this as part of your Plan of Care.
FAQs on Direct Billing with Momentum Health & Evidence Sport and Spine:
Is direct billing available for all insurance providers?
Direct billing is available for most major insurance providers. Please check with your provider to confirm if your insurance is eligible for direct billing. It may be helpful to ask what services they cover, what the annual limit is, and when the limit renews. Please note that products, like orthotics, do not qualify for direct billing.
Do I need to pay anything up front if I use direct billing?
In most cases, you will not need to pay anything up front if you use direct billing. Any portion of the cost not covered by your insurance plan will be billed to you after your treatment.
