Preparing for your first appointment at Momentum Health

We will have emailed you a link to a fillable intake form. Save time on your first visit by completing your intake paperwork online, prior to your appointment. If you do not have time to complete them ahead of time, you should arrive at least 20 minutes early to give yourself time to finish them before your appointment.

The Momentum Health team is committed to finding the root cause of your condition and developing a treatment plan that addresses your needs and helps you achieve your goals. Completing your intake paperwork, which includes a detailed past and current medical history, assists our clinicians in accomplishing this.

Your First Visit

On your visit, check in with the smiling office assistant at the front desk when you arrive. We’ll ask for your insurance information and any other paperwork you may have brought with you. You can upload additional documents (such as imaging or a printed intake form) on our location specific Online Portal. Be prepared to talk to your clinician about your medical history, previous treatments, medications, symptoms, functional limitations, and other relevant details.

When to arrive:
  • Plan to arrive 10 minutes before your scheduled appointment to find parking and settle in (20 minutes if you still need to do your paperwork).
What to bring:
  • Insurance information, including your insurance card and any necessary authorization
  • Although not required for most of our services, bring a referral form if you have one
  • A list of current medications, allergies, and any relevant past surgeries
What to wear:
  • Comfortable, loose-fitting clothing that allows for easy movement and provides access for the clinician to examine and treat the affected area. (Athletic wear or clothing that can be easily adjusted is ideal.)
  • Supportive, closed-toe shoes, such as sneakers, to ensure safety and comfort during the session.
  • Avoid wearing tight or restrictive clothing, which may hinder your ability to move freely during the session.

What to expect during your First Physiotherapy or Chiropractic visit

  • During your first session, you will get a comprehensive evaluation, a plan of care tailored just for you, clear education, appropriate pain relief and even some important home exercises.
  • The evaluation includes a review of your medical history, an assessment of your current condition, and an examination of your range of motion, strength, and movement patterns.
  • Our clinicians will evaluate your goals and establish a personalized treatment plan, specifying the duration and frequency of your sessions.
  • Based on your assessment, our therapist will outline the specifics of your treatment plan and provide relevant information to ensure you have a clear understanding on how it will be effective to your road to recovery.

Your first session will include some hands-on treatments to address these concerns, such as manual therapy or other treatments.

Your physiotherapist, chiropractor, athletic therapist or kinesiologist will likely also give you home exercises to support your progress in therapy.

After your first session, you’ll arrange the remaining sessions at the front desk before leaving. During those subsequent visits, we’ll build on the initial evaluation to create the most significant impact possible. Our goal will be to help you recover quickly and achieve meaningful, lasting results.
